I know that I have anxiety issues and before I went to the doctor and finally got diagnosed and put on medication my chest hurt SOOO bad I would be sitting down in class and all of a sudden I would be short of breath and my chest would hurt. My teacher even stopped the class to see if I was alright (it was rather embarrassing) I didn't think I was stressed or had anxiety or depression but we tried many things before this finally worked. KEEP TRYING to find an answer and keep pestering your doctors it is there job to help you out and if they can't maybe try talking with someone new. There has to be some kind of answer keep trying something has to turn up.

No idea-we're not doctors here. I say get a second opinion. You don't cough with your lungs hurting for no reason.
Do you work out a lot? Over exert yourself? Could be recurring Costochondritis (inflammation of the cartilage attached to the sternum).
Have you had a CBC (complete blood count) to check the levels of platelets, red blood cells, white blood cells, neutrophils, etc? Sometimes, a deficiency such as this can cause the symptoms you're having.
Do you have any allergies? Do you carry an epi-pen? It could be an allergic reaction.
Are you under any significant levels of stress during these attacks? Sometimes, psychological problems such as stress can cause a physiologic reaction.
