There's this pressure to look 'naturally' beautiful, and if you're willing to pay for it, there are many cosmetic procedures can help you get there, it just depends on how much "risk" you're willing to tolerate.
A person can talk about a procedure and still decide not to do it. Sometimes it's also just a litmus question if you'd be supportive if they did do it.

My nose didn't need plastic surgery. It's a perfectly fine nose.

No, I think she is asking for herself. She wants to know if you would see it as normal if she gets one, I guess. Or she is asking for your opinion on people who do get plastic surgery in general.
Depends why she asked. She might be planning some for herself and was only asking your preference without even thinking of any imperfections you may or may not have.
