How do you ask a girl out?

I would get a small group together for something & be like " oh hey some friends & I are hanging out & I'd really like you to come" or something like that. That way if she says no it isn't a big deal. But then again, you could just straight out ask her.

Possibly organise a party with your roomies/friends sometime and invite her and her roomies/friends along. The good thing about this is, they don't even have to know why you are having a party. Just for the sake of it in their eyes, they will probably invite their friends along too.

Catch up with her there in a relaxed, happy setting. Look her in the eyes and smile. Listen to what she has to say attentively and where possible ask her open-ended questions to what she tells you.

She'll probably be able to tell that you are the shy type - In fact, she may know this already from your time together at high school - so the fact that you made the effort to talk to her despite your shyness, will probably touch her and be a good indicator that you like her. Why not invite her along to something that you were already doing? For instance, if you're going to catch a movie on Thursday, ask her if she would like to come along.

If that doesn't work, there's always the direct approach. Just ask her to go somewhere with you. I know rejection can be a bitch, but you just gotta do it man!
